Geriatric Assessments/Evaluations

As we grow older, we are challenged by physical, psychological, and social changes that affect 1) ability to function, 2) safety and independence, and 3) satisfaction with life. Geriatric assessment provides an outpatient evaluation that identifies the individual’s strengths and limitations. A coordinated plan of care that identifies recommendations and available resources is provided at the completion of the assessment. A team of experts, trained to specifically identify the needs of older adults, can include a physician, nurse, social worker, and psychiatrist/psychologist. Additional members of the team may include a dietitian, physical, occupational, or speech therapists. The assessment may require anywhere from one to three visits, including a family conference. Medicare and secondary insurance cover a portion of the cost.

When would it be appropriate to consider a geriatric assessment;
• Difficulty functioning or performing activities of daily living
• Confusion or memory impairment
• Depression, loneliness
• History of falls or weakness
• Caregiver/family stress
• Nutritional problems
• Multiple medications
• Repetitive emergency room visits or hospital admissions

Benjamin Rose Community Services provides older persons (over 60) with needed assistance to remain in their own homes as long as possible and assists family and friends who care for them.

Services provided include:
Telephone consultation and information regarding resources.

In-home Case Management - Third party reimbursed or based on a sliding fee scale adjusted to income. (Nursing, Social Work, Personal Care, Homemaking, Shopping and Escort Services)
How can these services be obtained?
An Intake worker will help you determine what you need. If you live in Cuyahoga County and decide to use Benjamin Rose services, a nurse or a social worker will schedule an appointment to come to your home and work out a service plan with you. A social worker or nurse will be assigned as your Case Manager. The Case Manager is a required service of Benjamin Rose in order to monitor and supervise the services.

Outpatient social workers assist patients after discharge through home health, case management and community services, as well as clients and families living at home with health and age-related concerns.
Outpatient services provided by the Social Work Department include but are not limited to:
Assessment of psychological, social, environmental and financial impact of illness on adults and families.
Assisting adults, when necessary in identifying and transitioning to the most appropriate home care services.
Counseling regarding adjustment to illness and care giving, long term care and community services planning.
A bi-monthly caregiver support group.
Providing assistance with Advance Directives, Living Wills and Durable Power of Attorney for Healthcare.
Help with medication assistance programs.
The S.H.A.R.E. program to help patients organize their medical bills and insurance forms.
Community information and referral.

The Elder Evaluation Program is an outpatient geriatric assessment utilizing a multidisciplinary team. Program works with the patients primary care physician.

Gatekeeper Program - Identifies isolated and at-risk elderly through the use of non-traditional referral sources known as Gatekeepers. Gatekeepers are individuals who are in contact with older adults through their everyday jobs, such as police, paramedics, Meals-on-Wheels volunteers, senior center workers, etc., and are trained to identify problems that impact an older adult's safety and independence. Volunteer gatekeepers make a referral to the Gatekeeper Program when they have identified an older adult in need of assistance. Once the referral is made, a social worker and registered nurse visit the older adult in their home and conduct a comprehensive screening, thereby identifying the older adult's needs and assisting with their connection to services, allowing the individual to remain safe and independent in the community.

Oakview Behavioral Health Center provides comprehensive behavioral health services including both inpatient and outpatient treatment for mental health and addiction disorders. Oakview serves individuals ages 13 and older and offers specialized services to individuals experiencing both emotional and substance abuse problems at the same time. Oakview is a contracted provider with most major behavioral health insurance plans.
