OPTIONS is for those who are not eligible for the home support services through PASSPORT.
ELIGIBILITY:60+ years of age.SERVICES AVAILABLE:
Residents of Cuyahoga County.
Living in a private home or apartment (excluding assisted living residences, adult care homes, nursing homes, and group homes.)
Eligible for home support services based on an assessment of need for assistance with daily activities, such as bathing, dressing, food preparation.
Managing expenses on a modest monthly income with limited financial reserves.Medical transportation

Visit Our Web Site Click on Home CareIt is the mission of the Western Reserve Area Agency on Aging's PASSPORT program to support older persons who wish to remain in their own homes with independence and dignity.Eligibility for PASSPORT includes:
Being 60 years of age or older.Each PASSPORT participant has a Case Manager, a registered nurse, or licensed social worker, who coordinates home care services and maintains regular contact with the participant and PASSPORT provider agencies.
Being eligible for Medicaid.
Living in a home or apartment.
Needing hands-on help with daily activities such as bathing, dressing, and meals.
Agreeing to accept PASSPORT services.
Having basic health and safety needs met.
Maintaining a cost for PASSPORT services at or below 60% of the cost of nursing home care.Home services include:

Home-delivered meals
Homemaker/respite care
Home medical equipment and supplies
Independent living assistance
